Creating Templates
Build reusable consent forms, manage versions, and control what clients see.
Consent form templates are managed from the Digital Consent Forms extension.
Where to create templates
Go to Extensions → Digital Consent Forms → New.
Create A Template
- Go to Extensions → Digital Consent Forms
- Click New
- Write your content and add the input fields you need
- Save the template
- (Recommended) Set the template to Active when it’s ready for real clients
Editor & Inputs
Templates use a block editor.
Typing / opens a command menu where you can insert things like:
- Headings
- Quotes
- Layout blocks
- Input fields (name, date of birth, confirmations, etc.)
- Signature fields
Signature fields
If your template includes a signature field, the client must sign before they can submit the form.
Active vs Inactive
Templates have a status:
- Inactive: use as a draft or internal work-in-progress
- Active: use for templates you want staff to send
TatTool may still show inactive templates in some dropdowns. To avoid mistakes, studios typically only send Active templates.
Versioning (Editing Templates)
Consent form instances reference a template by ID. This means:
- Signed forms are unaffected by template edits (the PDF snapshot is already stored).
- Pending forms may reflect template edits, because the signing page renders the current template content.
Avoid changing templates after you’ve sent them
If you need to update wording for future clients, consider duplicating the template first, then send the new version going forward.

- Load/duplicate an existing template as a starting point
